CITES Help Desk NetID Password Change Forms

If you have forgotten your NetID password, you can reset it by faxing the NetID Password Change Form to the CITES Help Desk.

1. Print one of the following forms:

• NetID Password Change Form (HTML version)
• NetID Password Change Form (PDF version) Note: The PDF format requires Adobe Acrobat Reader.

2. Fill out the NetID Password Change Form.

3. Make a copy of two forms of photo-id (for instance, your driver's license and your I-Card).

4. Fax both documents to (217) 333-4368. Note: The fax number is also written on the NetID Password Change Form.

The CITES Help Desk will contact you when your NetID password has been reset.
